Dear Parents of Advanced Choir Students,
Your student will bring home a packet of forms that need to be filled out and returned to us. We are including links to soft copies below, for your convenience. All forms can be found online, in Charms, under Trips.
Please carefully read the Trip Regulations form.
IMPORTANT: THE FOLLOWING FORMS NEED TO BE ON HAND ONE WEEK PRIOR TO TRIP DEPARTURE IN ORDER TO ALLOW YOUR STUDENT TO GO ON THE TRIP, so please return them to us ASAP. Please remember to fill out the forms entirely, and sign where needed. Please keep forms stapled together.
- Pre-arranged absence forms: REQUIRED - The Pre-arranged Absence Form must be signed by ALL the students' class teachers.
- Field Trip Permission Form: REQUIRED
- Emergency Care: REQUIRED (Click HIDE INSTRUCTION BOX)
- Medication Authorization: required if your child needs to take prescription medicine
- CHS Music Dept Medicine and Prescription Authorization: required if you want to allow chaperones to give your child over-the-counter medicine or if your child needs prescription medicine. Please note that cannot give your child any medicine without this form, not even for a headache.
- Epinephrine Authorization: required if your child needs it
- Luggage Search: REQUIRED
