*** TURN IN ALL FORMS BY FRIDAY, MARCH 19 ***

Be sure that Pre-Arranged Absence forms are signed by ALL class teachers before you return them.

Place the completed forms inside the Brown Box in the Chorus room.

Links to the forms have been have been e-mailed to all students and are included here for your convenience. Use this as your checklist to be sure that you have completed all necessary paperwork:

Trip Regulations form - IMPORTANT that you read this form carefully

Pre-Arranged Absence form - ** REQUIRED ** - (Must be signed by ALL teachers)

Field Trip Permission form - ** REQUIRED **

Emergency Care form - ** REQUIRED **

Music Department Emergency Care form - ** REQUIRED **

Luggage Search - ** REQUIRED **

Medication Authorization form - (Required if your child must take prescription medicine)

Over-The-Counter and Prescription Details - Required if you want to allow chaperones to give your child OTC medicine or if your child needs prescription medicine

Epinephrine Authorization - Allows administering of Epinephrine to your child if necessary

Update #2 - ITINERARY
(March 14)

As we get closer to the trip date, be on the lookout for an e-mail with specifics on what to pack and what to wear at the different events. Please note that students will be responsible for paying for 2 lunches (see the detailed itinerary – red items), so they should bring $20 to $30 for those meals. They likely will want to bring additional funds for snacks and souvenirs.
